Acute Lymphocytic Leukemia

  • Patients with relapsed ALL or with high risk disease in first remission are offered an allogeneic transplant using a matched related donor
  • Selected patients in complete remission are offered an autologous transplant

Acute Myeloid Leukemia

  • Patients with poor risk AML are offered an allogeneic stem cell transplant in first remission if they have a matched sibling donor
  • Relapsed or refractory patients are offered an allogeneic stem cell transplant using a matched sibling
  • Patients without an allogeneic donor are offered an autologous transplant in first remission if they have poor risk disease or in second remission
  • Patients who are older than 60 years are offered reduced intensity conditioning and allogeneic stem cell transplant using a matched sibling “mini transplant”

Aplastic Anemia

  • Allogeneic transplantation from a matched sibling donor is offered to untreated patients or those who have failed ATG treatment.

Amyloidosis

  • Patients with amyloidosis are offered high dose melphalan conditioning and autologous stem cell transplant

Chronic Lymphocytic Leukemia

  • Patients with advanced/fludarabine-refractory/relapsed CLL are candidates for reduced intensity conditioning and allogeneic stem cell transplant using a matched sibling “mini transplant”

Chronic Myeloid Leukemia

  • Allogeneic transplantation from a matched sibling donor is offered to patients in chronic or accelerated phase
  • Patients who are older than 60 years are offered reduced intensity conditioning and allogeneic stem cell transplant using a matched sibling “mini transplant”

Germ Cell Tumors

  • Patients with recurrent or refractory germ cell tumors are offered high dose chemotherapy and autologous stem cell transplant

Hodgkin's Disease

  • Relapsed or refractory Hodgkin's disease patients are offered high dose chemotherapy and autologous stem cell transplant
  • Selective patients with relapsed or refractory Hodgkin's disease are offered allogeneic transplantation from a matched sibling donor
  • Selected patients with relapsed Hodgkin’s disease who are older than 50 years are offered reduced intensity conditioning and allogeneic stem cell transplant using a matched sibling “mini transplant”
  • Selected patients with relapsed or refractory Hodgkin's disease are also offered an autologous stem cell transplant followed by a reduced intensity conditioning and allogeneic stem cell transplant using a matched sibling “tandem transplant”

Multiple Myeloma

  • Patients with standard risk multiple myeloma up to the age of 70 years are offered a melphalan based conditioning regimen and autologous stem cell transplant
  • Selected patients with high risk multiple myeloma are offered reduced intensity conditioning and allogeneic stem cell transplant using a matched sibling “mini transplant
  • Selected patients with high risk multiple myeloma are also offered an autologous blood stem cell transplant followed by a reduced intensity conditioning and allogeneic stem cell transplant using a matched sibling “tandem transplant”

Myelodysplastic Syndrome

  • Patients with high or high-intermediate risk myelodysplastic syndrome are offered targeted busulfan based conditioning regimen and allogeneic transplantation from a matched sibling donor
  • Selected patients with high or high-intermediate risk myelodysplastic syndrome who are older than 60 years are offered reduced intensity conditioning and allogeneic stem cell transplant using a matched sibling “mini transplant”

Myelofibrosis

  • Selected patients with myelofibrosis are offered allogeneic transplantation from a matched sibling donor

Non-Hodgkin's Lymphoma

  • Relapsed non-Hodgkin's disease patients are offered high dose chemotherapy and autologous stem cell transplant
  • Selected patients with relapsed or refractory non-Hodgkin's disease are offered allogeneic transplantation from a matched sibling donor
  • Selected patients with relapsed non-Hodgkin’s disease who are older than 50 years are offered reduced intensity conditioning and allogeneic stem cell transplant using a matched sibling “mini transplant”
  • Selected patients with relapsed or refractory non-Hodgkin's disease are also offered an autologous stem cell transplant followed by a reduced intensity conditioning and allogeneic stem cell transplant using a matched sibling “tandem transplant”
